Autologous Conditioned Serum, a Novel Natural Remedy for Skin Aging; Review of Current Literature

Skin aging is a part of a natural human “aging mosaic” which becomes evident and follows different trajectories in different organs, tissues and cells with time. Skin aging is a complex biological process influenced by combination of endogenous or intrinsic (genetics, cellular metabolism, hormone and metabolic processes) and exogenous or extrinsic (chronic light exposure, pollution, ionizing radiation, chemicals, toxins) factors. These factors lead together to cumulative structural and physiological alterations and progressive changes in each skin layer as well as changes in skin appearance, especially, on the sun-exposed skin areas

Inflammation can be considered a core process of aging and vice versa, aging is sometimes referred as a chronic inflammatory state condition. Evidence has shown significant correlation between the aging process and: chronic inflammation and/or diseases with a subjacent chronic inflammatory state (age-related diseases).

Autologous Condition Serum (ACS) method, which is Exposure of blood leukocytes to pyrogenic free surfaces stimulates a set of anti-inflammatory cytokines, containing interleukin-1 receptor antagonist (IL-1ra), and several growth factors, in the liquid phase of blood.  IL-1ra is a competitive antagonist receptor of IL-1. One hypothesis is that the concentration of local IL-1 is elevated during skin aging and inflammatory conditions in multiple skin disorders. It is currently evident that other anti-inflammatory cytokines and soluble receptors, which demonstrate difference dissociation rates for IL-1α,IL 1β, and IL1ra, can influence IL-1 receptor signaling and inflammatory circumstances . The production of ACS rich in IL-1ra is conducted by incubation into a syringe of venous blood. The blood is recuperated and centrifuged after incubation for 24h at 37°C. This method stimulates a quick production of IL-1ra. Leukocytes produce increased levels of IL-1ra and multiple high doses of growth factors such TGFβ, FGF, PDGF, IGF, VEGF, MVP and IL8 and IL6 are accumulated in the serum.

In this article, we will review current evidences on skin aging pathology, novel biological anti-aging strategies with main focus on blood-driven products such as ACS and Platelet-Rich Plasma (PRP).
